Raising Concerns

Raising concerns is important to help ensure a safe and effective pharmacy workforce and minimise risk and harm to patients.

All submissions will be treated as confidential but it cannot be guaranteed that the originator will not be identified to the student concerned. Where a concern about an individual is progressed to a fitness to practise investigation, only in exceptional circumstances will your identity not be disclosed.

Any concerns you have in relation to an undergraduate or postgraduate pharmacy student studying at Queen’s University Belfast School of Pharmacy can be sent to us via a short form.

Reporting Persons raising concerns about a student’s fitness to practise are recommended (where possible) to complete and submit the Formal Statement of Fitness to Practise Concerns form.

Alternatively, concerns can be raised by emailing pharmacyconcerns@qub.ac.uk

Anonymous concerns raised regarding a student’s fitness to practise (including witness statements) will not normally be considered. Should, however, an anonymous concern raise issues of serious concern, the University reserves the right to investigate the matter and to take any appropriate action under the Fitness to Practise Procedure and Regulations. Any person who submits a concern must accept that any statement and/or evidence will be provided to the Responding Student.

Where a concern regarding a QUB student’s fitness to practise has been raised, the reporting person(s) will not normally be advised of any action or decisions taken under these regulations.
